1. Does Agency provide services to my community and how many years have they been in business?
2. Does Agency provide the type of services and caregivers that are needed?
3. What other services are available that might be needed later?
4. Will the initial assessment be scheduled at a time location convenient for the client and family?
5. How soon can the services begin and will the same person be providing care each time or day?
6. Does the Agency help with arrangements for other services and/or supplies that may be needed?
7. Are families encouraged to participate with developing a Plan of Care and are they involved with ongoing changes?
8. Does written Plan of Care clearly state services to be provided?
9. Does the Agency provide Medicare/Medicaid services? If not, will they accept private insurance payments for the type of services that are needed and Long Term Services?
10. Does the Agency provide information of who should be called with questions and complaints?
11. Is the Agency Licensed and Bonded to provide services in this State?
12. Does Agency charge a fee for initial assessments?
13. Does Agency have a Back Up Services?
